Such miniatures were made by jewelers and I am sure Suvorov 1st class miniature is certainly custom made. It looks like there's some sort of a loop on the back of small Suvorov to hang it on the ribbon.

For your information, there were two miniatures of the Order of Suvorov 1st Class for sale in the Red Auction (San Giorgio 22 Nov; 2008). One was with the righ ribbon, the second with a green plain ribbon. You can see them in page 50 of the paper catalog.
Their lot numbers were :
90 (with the right ribbon) : est. EUR 1.000/1.500. Sold EUR 1.000
91 (with the green ribbon) : est. EUR 200/300. Sold EUR 200
