I believe this is very interesting set.
Usually this badge is never numbered and does not come with the document.
My guess, that Leningrad Military District numbered limited number of the badges and issued the documents.
This badge and the document issued to the Major General for the participation in liquidation of accident on Chernobyl Plant.

Eric, it may be numbered just because of who it was given to - a major-general. Very interesting, both the badge and the document.
