It is not an official State level medal, so the numbering could've been easily officially sanctioned at some lower level.

It is still the right document. There was also a badge issued to the "liquidators" by Leningrad Military Region, which came with a document without any picture of the badge or any mention of it whatsoever. It is also not unusual for the Soviets to award badges to those who already have a certain document, for instance, a Partisan pin, that was given in the 1990s to those who already had Partisan IDs, issued in the 1950s and 60s.

I thought I'd revive this old thread and post this photo of an Air Force Captain (I think) wearing his Chernobyl Badge.
