Not sure what the literal translation is, but that is the "Земский начальник" badge, which is roughly translated as "land tract superintendent." Here's the (rough) Russian-to-English translation from a forum where I found some additional info:
According to the law of 12 July 1889 ("Regulations on land precinct captains), each county was divided into sections headed by land captains stood.
