What's strange with this one is not only can I not find someone with that name and age, I can't find one being awarded at that time (both 1945 awards) - or even an Order of that date and number. 058 of 15-09-1942.
A September 1942 award date would line up with the number fairly easy but an unscrupulous dealer may have added that information to the booklet and added the medal to make it a more appealing sell. Unfortunately there are some dealers that have started doing this in the last couple of years (central-eastern Europe) and this could be one of those. It does look eerily like one to me. It might sound biased of me but I think the best way to ensure this is what it states it is would be research by serial number.

I agree and would research this one. The document may be completely legitimate but two features catch my attention. The ink seems inappropriately fresh and the inordinate amount of information. The small entry areas and rushed administrative clerks usually resulted in much more scant information. It wouldn't be hard for a forger to marry a "naked" CSM with a period soldier booklet... My serial number data base puts this on the seam of Transcaucasus Front and North Western Front blocks.
