Table Medal "50 Years Of The Armed Forces Of The U.S.S.R.".

Here's a rather interesting document for the medal "50 Years of the Soviet Armed Forces". The recipient (an opera singer, interestingly) was also awarded the standard document that is normally found with the medal. Any idea what kind of medal would go with this document? It appears to be an official document for a medal of some sort...

The document is for a table medal, that is pictured in the document. The medal itself was missing from the group.

She had received her serie of military medals for singing at the front during the war.
