Here is a late UkSSR Document, issued during the early Post-Soviet period.

"Participant In The Liquidation Of Consequences Of Accident At Chernobyl Atomic Electricity Station" (Учасника Ліквідації Наслідків Аварії На Чорнобильській Атомна Електростанція) was created on the national, rather than all-union level on 18th June 1991, just 2 months and 6 days before the UkSS left the Soviet Union. Awarded for any period of work as a liquidator in 1986 and 1987, with work of at least 30 days in 1988, 1989 or 1990 also qualifying.

This was issued to Aleksandr Vjacheslavovich Mitrjushin by Donetsk Oblast on 15th December 1992.
