Now I got the first piece of research back.

It was awarded on 25.11.1966. It is no WWII catch up award !

It was awarded to a Starshina 25 years old born in 1941. În the Army since 09/1960.
UKAZ date in the award book is 22.12.1966, it was corrected to 25.11.1966 on award card.

No more awards. Only Ushakov medal.

So lets see what further research ......

досрочно и качественно ппо и ппр своего заведования. глубокие знания своеи специльности позволил ему правильно оценивать боеспособность технических средств и принимать верные решения.
в походе вахту нес бдительно и оказал большую помощь молодым матросам в изучении своеи специальности. в тяжелые минуты самообладания не терял деиствовал грамотно четко и хладнокровно.
проявил мужество стоикость и находчивость в сложных условиях плавания высокую бдительность в обслуживании деиствующих механизмов.
за отличиое вылолнение правительственного задания и проявленное при зтом мужество достони награждения медалью ушакова.
