It looks like one of the hundreds of Ukrainian "organisational" awards that are currently absolutely flooding the market. So many different designs, blank documents and high price tags emanating mainly from the east of Ukraine. Closer to "tourist" items usually than an award. Serious caution must be used when "awards" present themselves of which there are no state or ministerial decrees establishing.

It is a bit of a challenge finding comprehensive information on modern Ukrainian awards... I am looking for any good reference material.

Official state and ministerial awards are listed on governmental sites. A quick google search in Ukrainian should put you on the right track. From searches of online auctions more than 90% of those pretending to be real awards are just "organisational" bits. Chernobyl was the big Ukrainian "organisational" money maker until recently when the number of non-official awards has increased and then exploded.
Away from auction sites these "awards" can often be bought directly from the producers (often in Russia) for c$3-$4.
