Here is the Table Medal "Ural-Front 1941-1945". A very heavy and thick 60mm bronze Leningrad Mint item dating from 1983.
One of the finer Soviet Table Medals.

I would rather say it's "From Ural - to the Front". It commemorates the Factories helping the frontline by supplying it with equipment - here it's Tankograd - not sure if this "tank city" is indeed a city name or just a name of a factory?
