Scan on the left: it could be

meaning something like a point of transit of the Regional Military Comissariat in Lvov (Lviv).
I agree with Kirchgoens on Completed, the whole phrase means that the document was completed from his own words.
Second fragment on the right: misspelt, should have had складов НКО, i.e. related to warehouses of the People's Comissariat of Defense (Народный Комиссариат Обороны).
On the last fragment, it's a bit odd. Could be another case of bad writing for it seems that it is
383-я шахтёрская стрелковая дивизия, according to this entry in Russian Vikipedia:
