The Medal "For Merit" (Медал „За Заслуга“) was founded 25th December 1881 in three "metals" Gold, Silver and Bronze. Awarded either with or without a crown for civilian or military merit to the crown or to the nation. A special Gold Medal with diamonds was created for temporary regent Stefan Stambolov. The Gold Medal was awarded a total of 66 times.

This particular item was "liberated" during the Western allies push into Europe in 1945 so was most likely sat in a warehouse or the factory of production in the Greater German Reich rather than being awarded to a German by Bulgaria.
