I checked this against another Belarussian doc that was listed on the site recently and it actually looks pretty good to me:

I like that the stamp has the same distric name as the one written in. :thumbsup My only criticism would be on the penmanship, it is very proper and at times unsteady. It's almost as if someone gave the doc to their kid and asked them to fill it out. :) I think it's good though, and the matching medal doc is a nice touch.
Let me know if you'd like it translated, should be easy given the other translation.
