That's the Georgian Okrug.
This isn't listed on "the site" yet. I've noticed the Georgian issued ones are considerably more scare in the archive. This may be due to the political situation between Russia and Georgia or just chance but there are also some GSSR NKVD awards listed. I found a listing for this Medal to a Captain of Justice of the Military Procurator Troops Of The NKVD Georgian Okrug (ВП войск НКВД Грузинский округ). Hopefully there will be many more awards listed before they call it a day.

It looks like Kisha Davidovich Tshvediani (Киша Давидович Цхведиани) to me but not on "the site".

Defense of the Caucasus Medal document issued to Guards Senior Sergeant Gerasim Ivanovich Ochigava dated December 16th, 1946 however the difficulty lies in deciphering the issuing Commissariat, which appears to be Georgian judging by the stamp, as well as the title and signature. Any and all assistance appreciated, and thank you!

I just picked up my first Defense of the Caucasus document. I believe it was issued by a city military commissariat.

That would be Kirovograd to Guards Red Army Soldier Grigorij Kuz'mich Mamalyga on 24th July 1947. He was also awarded the Red Star and Medal "For Bravery".
