But what happened to these errors? Were they tossed into a refuse bin, or were they still awarded? If they were awarded out, I am sure that the numbers on the back would have still been used?

As you can see on the record card posted above, they were just awarded, and the erroneous serial number was put on the record card and order booklet.

Considering conditions in wartime Stalinist Soviet Union, there may have been draconian penalties for sub-standard work. It may have been easier for all parties involved to ship it out to the front if it passed visual inspection.

But if the award was issued, the number present is what would have been recorded, right? So in theory, the ORB with the 2 million number(of course inscribed in error), would have been recorded in the Orders Booklet and archives? It is not like the person issuing out the awards knows order sequence, right?

Exactly right.
If an Order says #4 a booklet will say #4 too if an Order says 4444444444 so will the booklet even if under 50,000 of the awards were ever handed out.
The numbers can be seen more as a sign of identification rather than counting the number awarded.
