Here is a document which reads;
"Identification #316
The bearer of this Rizhvadze, Demuram Omerovich indeed is a combatant of the extermination battalion "Fifth" of Regional Department NKVD Georgian SSR.
Head Of Battalion Podpis'
20th May 1942"
