I don't have this kind of document. The font looks Georgian. So, those GPW Ann. medals are awarded in:

Here are a pair of consecutivly numbered documents for the Georgian Medal to man (Nik'olozi Ivanes-dze K'azhanaia/ნიკოლოზი ივანეს-ძე კაჟანაია) and wife (Anna Grigoris-as K'azhanaia/ანნა გრიგორის-ას კაჟანაია). As these are issued and numbered in the late 46000 range and the document at the top is numbered 52000 range and is not issued we can presume that the Republic Of Georgia awarded c50,000 of these Medals.

#041013 to Nik'oloz Ivanes-dze Popovi (ნიკოლოზ ივანეს-ძე ფოფოვი) (Now reunited with other items), #063372 to Dimitri Vasilis-dze Beruashvili (დიმიტრი ვასილის-ძე ბერუაშვილი) and #063594 to Levani Dimitris-dze Gogichadze (ლევანი დიმიტრის-ძე გოგიჩაძე) (Also reunited), the latter two very much contradicting my previous post. Obviously more than 60,000 issued.

Here is #000333 to Nik'oloz Aleksis-dze Gadievi (ნიკოლოზ ალეკსის-ძე გადიევი). The only Nik'oloz Gadievi I can find was a founding member of the South Ossetian Revolutionary Committee in 1921 which would make him a very old man if this were the same person (90's-100's). There is also no Nikolaj Gadiev on podvignaroda which only adds to the mystery.

If it were awarded to the revolutionary his old age may have exasperated the award which has a rather early number of only 333 (out of many tens of thousands).

Here is #018848 to Ashot' St'epanes-dze Melkzhiani (ამოტ სტეფანეს-ძე მელქჟიანი) who, according to "the site" was born in Baku in 1914. A rather strange name as it is a Georgian spelling of an Armenian name which may have been translated via Russian.
