#12 appears to be a fake Civil War-era cavalry commander service cap badge

#13, #14 is "Excellent Health Care Worker" badge

#15 is "Ferrous Metal Industry Socialist Competion Winner" (or "Excellent Worker")

12 - it's not a cap badge - those were worn on the breast. not an expert to say if authentic.
15 - I really love the badge design! The badge seems to be called excellent worker of the industry as given by Desantnik, however the reason for awarding given in the doc says it was given for "Stakhanov Labour", so in this case, yes it was a socialist competition winner, but I don't know if this was the only reason for this badge to be awarded?
