Home of record: Azerbaijan ASSR, Kirovobat Station, Railroad Building #12, Apt. 5 Short description of personal combat feat or service Senior Lieutenant Kulevatov displayed bravery and heroism during fighting for Budka Station on 14th January, 1944. While attacking an enemy artillery battery, fire from his tank destroyed two enemy bunkers and their crews. When his tank became disabled, Senior Lieutenant Kulevatov commanded his platoon on foot while mopping up the enemy from the station. He personally killed five enemy soldiers with his submachinegun. On 16th January during the destruction of an enemy artillery regiment near Podberezvo Station, his tank’s fire destroyed as many as 20 enemy soldiers and officers. Senior Lieutenant Kulevatov is deserving of the Order of the Red Star for displayed heroism and bravery. Signed Battalion Commander, Guards Major Vlas(?)
