On behalf of the Presidium of the Supreme Soviet of the USSR, I award:

12. Senior Sergeant Nikolai Petrovich Yakovlev, assistant leader of a dismounted reconnaissance squad in the 1st Rifle Battalion, for the fact that he, while serving with a group of five commanders, conducted a reconnaissance mission in the direction of the village of Usvyatskiye Nivy on November 17, 1942.
The scouts spotted a group of six Germans, who were working on preparing defensive positions. The senior officer of the group, Captain Pershin, decided to attack this group and take a prisoner.
With contempt for death, Yakovlev attacked the enemy group, numbering 6 men, in a quick rush and under heavy enemy machine-gun fire. During this rapid operation Senior Sergeant Yakovlev showed himself a bold, brave, and decisive commander, who, with contempt for death, strived to assure the success of taking a prisoner. The mission to take a prisoner was accomplished: a prisoner was taken and delivered to the regimental staff.

Short, concrete description of the personal combat feat or merits:

During the fighting for the village of Kozadayevo on July 1, 1942 he personally crawled toward a German machine gunner, killed him, and picked up his machine gun, with which he shot down the fascists, who were fleeing in a disorderly manner.

His group accurately carried out the mission assigned to them, without suffering any losses. During the fighting he was wounded in his head, but he continued commanding his men until the end of the operation.
