Here is the Title "Republic Of Karakalpakstan's Honoured Health Care Worker" (Qaraqalpaqstan Respublikasi'na Miyneti Sin’gen Densawli’qti’ Saqlaw Xi’zmetkeri/Qaraqalpaqstan Respublikasına Miyneti Sin’gen Densawlıqtı Saqlaw Xızmetkeri/Қарақалпақстан Республикасына Мийнети Сиңген Денсаўлықты Сақлаў Хызметкери).

This Title is the seventeenth highest award of the award system of Karakalpakstan and was founded c1994. This is a Type 1 (Pre-2009 script).
Notice the die break on the coat of arms on the reverse which is a feature of all the Titles issued late in the production run (the same die was used for the reverse of all the Titles so the break is on all of some)

The Title is awarded sparingly and for the following merits;

"«Qaraqalpaqstan Respublikasi’na miyneti sin’gen densawli’qti’ saqlaw xi’zmetkeri» – densawli’qti’ saqlaw tarawi’n rawajlandi’ri’wda ha’m medicinali’q xi’zmet ko’rsetiwde ja’ne respublika xalqi’ni’n’ densawli’g’i’n saqlawdi’ sho’lkemlestiriwde ayri’qsha xi’zmetleri bar, ilimiy-izertlew, yemlew-profilaktikali’q, sanitariyali’q-epidemiologiyali’q ha’m farmacevtikali’q ma’kemelerde, a’dette keminde 15 ji’l islegen xi’zmetkerlerge, soni’n’ ishinde worta ha’m kishi medicina xi’zmetkerlerine beriledi." (15 years service)
